Policies & Forms
Learn about the policies that help us provide your children with the very best of care.
Please call the office to make an appointment or speak with us. Whenever possible, we do our best to accommodate acute care visits for Leap patients.
If you miss an appointment without calling us, you deprive other patients of the chance to take that appointment slot. For that reason we require at least one business day for all cancellations.
If you are going to be more than 10 minutes late, please call to see if we can accommodate you. You may need to reschedule your visit.
If you are late to 2 appointments or you miss 2 appointments we will discuss whether and how we can accommodate you.
If your child’s illness or injury is life threatening, dial 911.

24/7 Nurse Phone Lines
For urgent matters that cannot wait until office hours, consider reaching out to your health plan’s nurse line for advice, or check our trusted online resources. If your child needs urgent medical treatment, please don’t hesitate to go to an urgent care or emergency room. Your child’s well-being is always our top priority.
If a patient needs hospital care, we direct them to M Health Fairview Masonic Children’s Hospital or a hospital recommended by their health plan.
We are proud to care for people who have these insurances and those who have straight Medical Assistance coverage.
If you want us to take care of your child, but you have another insurance, please call us and we will see if we can apply to take your insurance also.
